Emerging Lessons from Clinical-Community Partnerships in North Carolina

Close to a decade ago, the Blue Cross and Blue Shield of North Carolina Foundation launched the Community-Centered Health initiative. This initiative focuses on increasing the capacity of community-based organizations and health care entities to address the root causes of health and health inequities. By providing support for cross-sector partnerships in North Carolina communities, this long-term initiative aims to center the perspectives of those most impacted by inequities to identify barriers to good health and implement approaches for overcoming them.

Engage R+D partnered with the Blue Cross and Blue Shield of North Carolina Foundation on learning and evaluation activities for the Community-Centered Health initiative. This retrospective evaluation report examines progress and enduring outcomes from the first cohort of three Community-Centered Health partnerships. The report also highlights lessons about supporting equitable community-driven change that have relevance to other funders and communities engaged in similar work.
